MEIFHA 2024 - 2025 Membership Survey

Now that the 2024 - 2025 season has wrapped up, your MEIFHA Executive would appreciate your anonymous feedback about this season. This information will help us gain valuable insight as to how the overall experience might be improved for players and families alike.

Remember to complete the survey for ALL coaching staff and Team Volunteers so we have feedback for Head Coaches, Assistant Coaches, Managers, Treasurers, etc., for next season. 
 
If you have players on multiple teams please fill out a survey for each player.
 
We do not collect emails on submission;  Feedback is anonymous.
